A 6-Week Physician-Referred Wellness Program

Prescribe the Y is a structured, 6-week wellness program designed to help individuals safely increase physical activity, build healthy habits, and improve overall well-being using YMCA resources.

Through weekly group sessions led by YMCA wellness professionals, participants receive guidance, accountability, and education to create sustainable lifestyle changes. Our goal is not quick fixes — it’s long-term health.

Step 1: Physician Referral

Participants must obtain a referral from their healthcare provider indicating they are cleared for moderate physical activity.

Step 2: Enrollment & Baseline Assessment

Participants attend an initial session that includes:

Participants attend one 60-minute session per week that includes:

  • Guided discussion and education
  • Structured movement instruction
  • Accountability and progress tracking
  • Weekly action plan and homework

Step 4: Final Assessment & Transition Plan (Week 6)

Participants complete final measurements, reflect on progress, and develop a plan for continuing their wellness journey at the YMCA.
